Heres a pretty decent list. Also, a game clip and I like to have my flare gun stored within my kayak. You never know!

absolute bare minimum?

yer boat and a pole, net, or spear...like back in the day.
or like kevin pointed out that checklist is a great start.
can add: anchor, drift bags/drogue, hat, shades w/ gators, dry bags. oh and certain special security tokens i frequenlty hear mentioned here!

My absolute bare bones necessities are:

Paddle with leash
PFD
VHF
Rod and Reel with leash
Plastic and/or Iron
Sun protection
Water or similar drink
Pliers
Bilge Sponge
Your licenses

A couple of additions that the DFG require but are rarely checked are:

Net with an 18" opening
Some type of stringer (or is that freshwater only?)

Off shore?? What do you mean by that? Out side the kelp beds in LJ or hunting paddies in the summer? Long paddles to lonely seamounts in baja? A bay to bay trip? Not trying to be a ahole.

Plenty of water.
In season; sun protection or warm gear.
Radio.
Experience learned from many shorter trips.
PFD! On your body and not in the hatch or strapped to the back!!

Most importantly for off shore or remote areas; Have a buddy with you.
